To enter, simply register for KS95’s Clouds Choir for a Cause, or send an 8½ x 11” sheet of paper with your complete name, address, email address, and two phone numbers (identified as daytime and evening) and mail in a first-class stamped envelope to:

No donation is necessary and a donation will not give you a greater chance of winning. Entries must be received at the above mailing address by December 6, 2024. Children’s Cancer Research Fund is not responsible for the delay in delivery of entries for any reason.

The winners will be chosen by a random drawing to be conducted on December 9, 2024 from all entries received by the deadline. The odds of winning are dependent upon the number of valid entries.

The winners will be notified by a staff member of Children’s Cancer Research Fund via telephone to the daytime phone number provided by the entrant on the entry form. A message will be left if there is not an answer, with a return phone number given. If there is no answer to the initial call or message by the end of the next business day, a second phone call will be placed on December 13, 2024 and an email will be sent at the same time to the email address provided by the entrant.

The winner will need to reply to and send a message back via one of these communication channels by 12:00 p.m. (CST) on December 16, 2024 in order for the prize to be claimed. Winner’s failure to respond to the message shall be deemed forfeiture of the prize and an alternate winner may be selected at Sponsor’s sole discretion.

There will be a total of one (1) winner. Each winner will receive one (1) Roku Pro Series 65” television and one (1) Roku Pro Series 65” mount.

Restrictions: Winners are solely responsible for any taxes associated with receiving the prize.

Prize and Approximate Retail Value (“ARV”) – One (1) Prize Total: The prize is a Roku Pro Series 65” TV, model 65R4A5R (ARV: $1199.99) and a Roku Pro Series 65” mount, model 65R4A5R (ARV: $99.99). Television is subject only to the manufacturers’ warranties and guarantees. Sponsor has no

responsibility or liability for any product malfunctions. Prize consists of only those items specifically listed as part of the prize. No cash or other substitution allowed by winner, but Sponsor may substitute the prize, or any portion thereof, for a prize of equal or greater value at Sponsor’s sole discretion for any reason. All local, state, and federal taxes on the Prize are winner’s sole responsibility. The winner is responsible for applicable taxes and fees, if any. The prizes are non-assignable and non-transferable. Sponsor reserves the right in its sole discretion to substitute the prize for one of equal or greater value, for any reason. A 1099 for the total amount of the prize will be submitted to the proper taxing authorities

Employees and non-employee workers of Children’s Cancer Research Fund (“Sponsor”), KSTP-FM, LLC (KS95), Hubbard Radio, LLC, TCL and their respective parents, subsidiaries and affiliated companies, advertising, promotion or production agencies, and their immediate family members (spouse, parents, children, siblings, and each of their respective spouses, regardless of where they reside) or individuals residing in their same household (whether or not related), are not eligible to participate or win. Void where prohibited.

The winners agrees to release and indemnify Children’s Cancer Research Fund, KSTP-FM, LLC, Hubbard Radio, LLC and TCL including their officers, directors, employees and subsidiaries, affiliates, parent or related companies from and against any and all liability, claims, damages and actions arising directly or indirectly out of or sustained in connection with the awarding, acceptance or use of the prize, and consents to the use by the sponsors of this contest for promotional purposes only of his or her likeness and/or voice and address (city and state) without any additional compensation. By accepting the prize, the winners agree to be bound by these official rules, including the above release.

Decisions of Sponsor concerning any and all matters with respect to this sweepstakes shall be final. Sponsor reserves the right to change these rules, as well as the terms and conditions of this sweepstakes, without notice. Sponsor reserves the right to change the prize at any time, or to substitute prizes of comparable value. Sweepstakes void where prohibited or otherwise restricted by law. Sponsor is not responsible for printing errors in the sweepstakes materials or these rules. If Sponsor determines in its sole discretion that the integrity or fairness of this Sweepstakes has been corrupted in any manner, Sponsor reserves the right to terminate the sweepstakes and to select a winner from among entries legitimately received prior to termination.
